For the purposes of processing your personal data we are the Controller.

The Personal Data We Process and What We Do with It
From the time of your initial enquiry, through your assessment, to completion of your treatment, we must record information about you.

This may include:
Your name, address, telephone numbers and email address;
Date of birth, general health, past medical history, and current medications;
The history and symptoms of your current problem;
Activities you undertake, employment history and goals as relevant to the problem that you are seeking help with, and the treatment of it.
The recording of the results of the physical examination, diagnosis and treatment responses.

Sharing Your Personal Data
We may share your information with other medical professionals if we consider it to be in your best interest. We will not share it with any other third party except with your explicit consent.

Retaining Your Personal Data
Whilst you are a client of ours we will continue to store and use your personal data. We are also legally obliged to retain this information for 8 years after your last communication with us – or if you are under 18 when you last see us, we must retain it until your 26th birthday. We will destroy all information within 1 year of this date.

Your Rights
As we process your personal data, you have certain rights. These are a right of access, a right of rectification, a right of erasure and a right to restrict processing.

Should your personal data that we control be lost, stolen or otherwise breached, where this constitutes a high risk to your rights and freedoms, we will contact you without delay.
